Community Support Professional I - Housing Specialist

Responsible for implementing the Permanent Supportive Housing program.
Works collaboratively with designated NRVCS staff, other agencies, and service providers to develop and provide stable, affordable, and long-term independent housing opportunities through short and long-term subsidies such as Section 8 vouchers or other types of permanent housing.
Provides assistance to adults with serious and persistent mental illnesses, including homeless adults.
Starting Salary:
$37,149-Depending on ExperienceKey Responsibilities Responsible for scheduling, coordinating and facilitating housing opportunities for individuals through housing development activities.
These activities include providing outreach to landlords and consumers to market the Permanent Supportive Housing program, conduct face-to-face housing presentations, and help complete applications of various rental housing resources including print media and Internet sources.
Assists individuals with activities to secure and maintain affordable housing such as contacting prospective landlords to schedule showings of rental units; scheduling housing interviews for consumers; assisting with the rental application process and help individuals make informed decisions; doing periodic home visits Assists individuals meet tenancy obligations such as timely rent payments and other lease requirements.
Including to help mediate landlord-tenant issues; Assist individuals with housing-related issues relevant to fulfilling lease requirements such as initial household setup, budgeting, housekeeping, house rules, contacting the landlord about repairs and other problems in the home, health and safety (including fire emergency plan and other safety and security concerns), and other issues.
Develops and maintains appropriate co-occurring program materials.
Tracks compliance and success of the program individuals, provides individual progress reports to and for the program individuals in order to ensure services are appropriate and identified needs are being addressed.
Assists with crisis resolution and general problem-solving.
Ensures services are provided with respect to the dignity of the individuals served as outlined in the State and NRVCS Human Rights Plans.
Advocates for services on behalf of individuals served and their families/care givers as needed.
Documents and reports suspected violations of Human Rights according to NRVCS policies.
Establishes and maintains comprehensive system for referral and networking of client's service and resource needs.
Works to establish linkages between the client and naturally occurring supports in the family and community, as well as with other service providing agencies/organizations (e.
g.
, Departments of Social Services, Health Departments, Virginia Department of Rehabilitative Services, Community Action, Probation & Parole, etc.
) Coordinates transportation resources as needed to access services.
Assists with completion of applications and assessments for services and resources as needed.
Maintains regular contact with clients, medical treatment providers, and other support systems and service providers as needed to implement the client's ISP and maintain stable housing.
This includes correspondence and phone contact.
Maintains complete and accurate records, turning in all required paperwork (i.
e.
:
clinical documentation, time sheets, mileage documentation, incident reports, leave requests, etc.
) on time.
Stays informed about and follows all NRVCS and funding sources' policies and procedures.
Performs at the expected levels of individual and team productivity.
Ensures all client records are in compliance with applicable local, state, and federal requirements including guidelines as they pertain to DBHDS Licensure Regulations, Waiver, State Plan and Clinic Options Medicaid.
Maintains all communications, both written and verbal, with respect and confidentiality and complies with HIPAA and 42-CFR Part 2 standards and regulations.
Promotes positive communication and collaboration among team members and other community resources needed to provide quality services.
Conducts self in a professional manner at all times.
Participates in team meetings unless absence is excused, completes proper written communication and relays other helpful information as necessary by voice mail and/or e-mail.
Serves on designated teams, committees and work groups.

Qualifications Requires demonstrated knowledge of case management/social work practices and resources available to help individuals with substance use disorders, including individuals with co-existing mental health disorders, and their families/care-givers in the community.
Verification of Qualified Mental Health Professional status Must possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record.
Must maintain acceptable record throughout employment.
Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment; Must have acceptable drug screen results upon hire.
Working knowledge of best-practice treatment modalities and methods for serious mental illness and possible co-occurring substance use disorders.
Basic knowledge of human rights, ethical, and legal concerns as they relate to providing case management in clinical and residential services.
Demonstrates initiative and the ability to work individually and as a part of a team.
Education/Experience Required:
Graduation from an accredited college or university with at least a Bachelor's Degree in Special Education, Social Work, Psychology, or other approved human services related field is preferred.
Will consider a high school diploma with one year of experience.
